Grotesque Fish is a quest within the game Dredge.
Caught a strange looking fish — never seen anything like it in all my years. It died as soon as it was pulled from the water — but its scales are still pulsing with a strange rhythm.
Don’t want to handle it any more. Have a headache and ears are ringing slightly. Maybe the Fishmonger at Greater Marrow will know more.

I should take the fish to the Fishmonger — he might know more about it.
Every fish that you can catch has an aberration form to it–sometimes more than one. If you do happen to catch a weird looking fish, then this quest will automatically pop up. While a lot of these fish can be random, there’s a higher chance of catching one if a school has an odd glow above it.
Head over to the Fishmonger. In exchange for the aberration, you’ll get a Handkerchief that was in the stomach of the specimen.
Someone will speak to you regarding it.
Meet at Blackstone Isle
A man approached me, interested in what the Fishmonger pulled from the fish. He wants me to meet him on Blackstone Isle.
Head south to the island with a large mansion on it. If you don’t have the Handkerchief on you, just look into your storage on the island, as storage is connected through everywhere and place it into your inventory.
Interact with the Ruined Mansion and speak with the man. In order to find the relics he’s looking for (which will have pink backgrounds when you do find them), you’ll need to fish through the dredge spots. You don’t currently have the appropriate equipment to do so, so he automatically installs it onto your boat free of charge.
