The Blood Fever is a quest within the game Assassin’s Creed Odyssey.

Talk to sick villagers
Head over to where you’ll talk with Phoibe. She’ll mention how her friend and friend’s family is sick with some sort of blood fever (because that’s really what you want to get involved with) and she requests you to help them.
Oh, you’ll help them all right–one way or another.
The family is being held captive toward the southwest by a few people who are trying to cleanse the are of the plague. Approach and interact with them for a conversation. Also note that you’re able to kill them before speaking with them, so be careful with the assassination prompt.

If you choose the first option, then you’ll initiate combat with the three men and end up saving the family, much to the happiness of Phoibe who wouldn’t want it any other way. Unfortunately, it comes at the price of a whole lot of deaths behind the scenes, in which case the family is going to die anyway.
In fact, if you allow the blood fever to spread across Kephallonia, it’ll be mentioned in future sections of the game, just to rub it in.
If you choose the second option, then you’re allowing the death of the family. But in doing that you’ll stop the spread of the blood fever throughout the entirety of Kephallonia.
So while Phoibe might be a little upset, and you might feel a little bad, the many lives saved outweigh the few that weren’t.
Can you cure the blood fever?
There is no cure for the blood fever, so if you chose to spare the family and come back to the island, you’ll find it’s population is pretty much decimated thanks to your decision.
