This guide is a work in progress

This is going to be one hell of a guide, so settle down and put on your glasses so you can read this shit, all right?

Great.

Now, I’ll do my damnedest to try and get you everything you need in the game, but I won’t make any promises–specifically with all the spirits, because getting all of them is a feat in and of itself. I also won’t be pointing out all the food items you can grab or coins because…you’ve got eyes, quite frankly and I can’t hold your hand for everything.

All the links in this guide will take you to screenshots of specific items, so no worries about outbound products you might find in my other guides.

If you’re interested, here’s a link to my straight-forward main game walkthrough that doesn’t feature any side quests.

So, in a nutshell:

  • This guide will probably not get you all achievements
  • I will not mention food locations or every single spirit location
  • I will not point out every written thing (they don’t count toward achievements)
  • Links will take you to screenshots

Also, you’ll need to have finished the game once, load a new game, and equip the transmission beads, I think? Just so you no longer have to use a phone booth for spirits.

Enter the hospital.

There are two Visitors that you’ll see first—a Rain Walker and Paper Doll. Wait a moment and there’s another Rain Walker that shows up from the hall to the left. Once they’re gone, follow the hallway to the elevator. (I won’t be mentioning anything you might be able to come across, as we’ve already been through this hospital at the start of the game in Chapter 1: Beginnings — City of Shadows — Mari’s Room.)

The only floor you’re able to go to is 4, so might as well choose that.

Ghostwire Tokyo Haunting Visions

Follow the hall to the left and enter room 403, where you’ll hear soft crying. Inside there’s a girl, but first, check the table on the left for a Paper Crane. Now speak to the girl about how she keeps seeing something scary when she closes her eyes, after she’s had a cornea transplant. That something is probably going to be found on the third floor.

Leave the room and turn right until you see a bright green hall when you turn the corner.

Before you burst your way through the doors, when you’re close enough, three Rain Walkers will spawn–(there are already a couple a floor below you before you approach the door). Bully your way through them and take the stairs on the right at the end of the hall.

There are two Rain Walkers, and one of them is super close to the steps. Go down too fast and you’ll be spotted pronto. One more is meandering along the area beyond the sliding doors.

Through the kitchen, there’s a door straight ahead (ignore the Visitor at the far end for now).

Inside the office are Shisa Statues on the top shelf to the left. Behind you on the other side of the curtained divider is a green book titled Kenta Owada – Fatal Accident Report and a password at the bottom.

Kenta Owada – Fatal Accident Report

Kenta Owada was admitted with a fracture in his right femur. Prior to his death, his treatment was progressing well and he was close to being discharged.

However, in the early hours of July 23, his body was discovered in an operating room. Cause of death is unknown; no signs of external trauma were found. As Owada was a registered eye donor, we are currently inquiring about patient in need of corneal transplants.

At the time the body was discovered, there were reports of some extremely strange things happening in the OR. Given the large number of similar eyewitness accounts, access to the area will be restricted for the time being, and the door will be locked with a passcode.

Passcode: 74398.

There are two other notes you can find in the office at the main desk titled Note to Hospital Director and Nurse’s Call Notes.

Note to Hospital Director

When I found Owada in the operating room, before he died he kept muttering about how he could see some woman. Then I saw her myself. It was a terrifying sight to behold. She was like some kind of shadow, and she looked like she was wearing a mask.

Please, you need to lock that OR. I’ve seen the shadow so many times since that day, and I know I’m not the only one. The room needs to be locked up before the patients start talking.

Nurse’s Call Notes

August 20

Misaki, the young corneal transplant recipient, seems to be having trouble sleeping. Every night she uses her call button to tell us that she sees a “scary lady” when she closes her eyes.

We think it must be some kind of post-op delirium. If her symptoms persist, it might be best to bring in a psychiatrist.

Now you can go after the Rain Walker to the left–but make sure he’s still at the door, because he was staring right toward me for some reason when I first walked out of the room. It’s funny because if you’re right behind the Visitor in order to Quick Purge, you’ll actually stand up and interact with the door lock. Luckily, the guy doesn’t realize it, but it’s still a what-the-hell,-man? moment.

Through the doors, the hallway will change toward the end.

Keep going and you’ll see a shadow to the right.

Go into that section and then through the hall connected to it. There’s someone to talk to at the end that’ll mention seeing the woman, making him the initial donor. In order to fight the Kuchisake, just start walking down the hall again until you turn around automatically.

Ghostwire Tokyo Haunting Visions

After she’s taken care of, the scene will change and you can speak with the ghosts in order to complete the quest and absorb the 3 spirits.

Leave the building.

Pillar of Light (part 5)

Northeast from hospital, there are spirits. Directly behind the gas station, there’s a jizo statue to maximize your water ether.

Directly south, grapple the tengu and look down to the left for a Rugged Walker and spirits. Re-grapple the tengu and make your way along the buildings to the southeast for a group of 3 spirits.

Climb on top of the highest point on the building you’re currently on and glide southeast again for a stage with three Students of Misery hanging out.

There are also 2 spirits and a Bubble Era Folding Fan sitting on the stage and graffiti on the stage wall.

Jump up to the northwest and up ahead are spirits as well as a Rain Walker and Paper Doll. Climb up onto the high billboard and travel north to a spirit, and then further north for 3 spirit groups. Three Wanderers will appear, but most importantly, don’t you forget the tanuki that’s posing as a traffic cone on the ledge of the building.

Now hop down to the spirits northwest.

Northeast, in the distance, you can see a few spirit groups, but when you head over there, you’ll actually be getting 4 spirits. Drop down to the west for spirits, and then turn left for a Tetsubin inside a storage locker.

If you look northwest, you’ll notice a tengu on the left and spirits on the right. Head over to grapple the tengu, and then scale your way up the buildings to get those spirits.

Glide to the building to the southwest for spirits. Now travel to the northwest for 2 spirits.

Another single group of spirits are northwest.

Drop down on the north side of the building you’re on for spirits. Directly to the west you’ll come across two Rain Walkers and a Rain Slasher. They seem to be guarding two spirits.

Things are a bit sparse around you right now, so walk a little to the east until there’s a supermarket on the right. Directly ahead you should see some spirits. There’s a metal fence you can climb up on in order to get them, otherwise you can kill the Rain Walker that’s nearby, and then jump the fence.

Make your way to the quest Rokurokubi, but before you get there, you’ll notice a blue mark in the screenshot on the left side. The bathroom has some graffiti on it you can examine.
