You obtain Johnny’s Broken Heart achievement in Chapter Two by going through the doorway to the left of the recording sign, entering the door on the right, and playing the pipe organ 5 different times.
Johnny’s Broken Heart Achievement
In order to be able to obtain this achievement, you’ll have to first get to the point where all of the ink puddles on the ground morph into monsters. Once they do, you need to kill all of them on the main floor, and once that’s done, the door blocking off the left side will open.
At that point, you can go through the hall and open the first door on the right.
Within is a pipe organ you’ll need to play five different times. At the end of each tune, you’ll hear a voice in the distance, kind of like moaning or sobbing.
